Paul Harris from Airedale Hospital NHS Foundation Trust asks:

“I am in the process of producing a suicide prevention policy for our OH department (rather than a policy as part of the wider wellbeing policy of the Trust).  The aim is to make sure that all clinical staff are assessing and appropriately documenting suicide risk assessments.  Does anybody have a similar document that they are willing to share?"
